Family Measurement Tools

Color coding shows risk categories from green for normal or optimal readings to red for a high risk category.  The other two colors used are blue and orange, showing movement away from wellness or green.  Family members with red, such as below, need to create an Action Plan with their doctor to move back to healthier measurements.

If your company provide this benefit, your company name and logo can be printed on the cover of their wellness plan.  This reminds employees who provided it, and creates an additional presence in the doctor’s office. (See example)

Individuals only learn if they are interested in a subject.  The individual user’s name on each tool increases the ownership of the information.  “This is about me, I have to do something about this.” is one reaction.  After creating a wellness program for a relative, the reaction might be, “My Mom’s blood pressure is coded red (i.e. Stage 2 Hypertension).  I will help her lower it.  (See example)
