Right Time and Place
Currently many entities recommend each of us become better integrated into our own health care team and do the same for our older relatives. (see quote to the right)
Information is needed when and where decisions are made - or much harder to see - when decisions are not made.
To be an effective member of your own health care team, you need to be armed with information, the questions “Why?, What does that mean?, What is my goal?, What can be done?”, plus the determination to not give up asking questions until you understand your circumstances.

Power and Presence

Unless you have a perfect memory, it is more useful to write down information or instructions in your own words in front of a medical professional so it will not get misplaced. Next confirm the correctness of your understanding. Because it is tied down inside your binder, it will be there for you to review when you have forgotten part of it and caring by your loved ones is easier too.
Is Everyone on the Same Page?

The quality of a conversation at the kitchen table, eye to eye, can be totally different than one done standing side by side at a computer. A determined look from you tells your loved ones you will help them succeed at getting better health outcomes.
Visual clues or consistent monitoring can increase your rates of success. A paper tool can be put on a bathroom mirror or refrigerator door to remind you about decisions that you have made and want to continue making. When the tool is filled in, just pop it back into your binder and start a new one. The old one reminds you of your successes.
